This form collects the information needed to enter a PEIMS-reportable contracted service provider into the district ERP system for assignment of a district email account, access to necessary applications and programs, campus location badge access, and other required resources. The service provider performs services in support of district operations or student programs as outlined in an approved contract and works under district or campus supervision. Please complete all fields accurately and submit electronically to Human Resources for system entry and access setup.
