Special Education Transition Facilitator

REPORTS TO: Director of Special Education; DAYS/DATE(S): 197; Teacher Pay Plan

Primary Purpose

Facilitate the overall implementation of transition planning and services, assuring that coordinated outcomes are appropriately planned for all students with disabilities. Provide parents and teachers with appropriate transition resources necessary to effectively plan and deliver transition services.

Qualifications

Education/Certification:

• Bachelor’s Degree from an accredited college or university
• Master’s Degree preferred
• Valid Special Education Texas teaching certificate
• Valid driver’s license with driving record that meets the district’s requirements

Experience:

• Minimum 3 years teaching experience in special education
• Previous or current experience working with special populations and/or transition services
• Minimum 2 years experience reviewing, developing, and implementing secondary IEPs

Major Responsibilities and Duties

• Serve as SCUC ISD’s Transition Designee as required by the State.
• Participate in transition planning for special education
• Train campus staff on transition and compliance
• Work individually with campus staff, as needed, in transition
• Provide job site coaching for students, as needed
• Consult with Special Education Teachers regarding the Community-Based Vocational Instruction
• Coordinate instruction and evaluation of student performance, with respect to transition, with other SCUC ISD staff
• Act as a liaison with parents to connect students with various community agencies that provide support to students with disabilities
• Submit all required paperwork in a timely manner and maintain records for future reference, as required
• Train and provide guidance to campus staff in preparing for ARD/IEP meetings when information about vocational preparation will be discussed
• Consult with Special Education Teachers and provide support/guidance for prevocational skills and the Community-Based Vocational Instruction Program
• Maintain documentation as required for compliance with timelines and state/federal requirements
• Serve as a resource for professional development for campus and district staff on training students with disabilities on disability awareness, self-determination, and self-advocacy
• Travel to campuses, work sites, agencies, conferences, etc.
• Support the mission of the school district by serving as a positive role model for students
• Consult with campus staff regarding the development of appropriate transition/graduation plans leading to successful post-secondary outcomes and positively impacting the district completion rate for students with disabilities
• Collaborate with parents and campus staff to facilitate agency connection and access to community resources
• Create and deliver professional development related to transition topics.
• Participate in staff development trainings, faculty meetings, and special events as assigned
• Maintain confidentiality.
• Other duties as assigned.
