OCCUPATIONAL THERAPIST

I. General description:

To provide occupational therapy to identified students. Supervises: Certified Occupational Therapy Aide/Assistant (C.O.T.A.) (if applicable)

II. Qualifications:

• Current American Occupational Therapy Certification
• Board or National Board for Certification in Occupational Therapy, Inc. certification and Current licensure from the Arizona Board of Occupational Therapy Examiners
• Must possess a Valid AZ Department of Public Safety fingerprint clearance card.
• Must be able to pass a background check upon employment

III. Duties and Responsibilities:

• Screen and evaluate appropriately referred students within the State and Federal requirements.
• Attend and actively participate in all appropriate Child Study Team (C.S.T.), Section 504, Multidisciplinary Evaluation Team (M.E.T.), and Individualized Education Program (I.E.P.) meetings.
• Develop occupational therapy related IEP/504 goals, objectives, and/or benchmarks, including any appropriate accommodations for qualified district students as assigned.
• Complete all State and Federally required paperwork within required timelines.
• Provide individual and group occupational therapy services to students as described in the IEP and 504 Plans.
• Prepare quarterly, or more frequent if needed, progress reports on each assigned student to include in the quarterly grade reporting process.
• Complete all MEDICAD reports and requirements in a timely manner.
• Participate in District training as appropriate.
• Perform other related duties as may be assigned by the Director of Special Services.
